Data is obtained from the Death Registration application form provided by funeral director and completed by spouse, partner or relative of deceased and Medical Certificate of Cause of Death and if a perinatal death, a Perinatal Supplement (to cause of Death Certificate), supplied by the certified medical practitioner certifying the death or supplied as a result of a coronial investigation |

The Australian Coordinating Registry (ACR) has the approval of all Australian Registry of Births, Deaths and Marriages (RBDM) offices (the data custodians) to provide services accessing Fact of Death and Cause of Death Unit Record File (URF) datasets on their behalf to an approved agency or applicant. Fact of death data is a compilation of death records from each of the data custodians. These files contain full name, date of birth and residential address details of all the people who have died in Australia. Cause of death unit record file is a snapshot of data that is compiled and coded (ICD-10) by the Australian Bureau of Statistics (ABS), based on data from the data custodians. This data set is used for statistical and research purposes. |
