Birth Registrations

Description: Permanent records of all registered births.

Cancer Registry

Description: Cancer case-based registry in which all notifications relating to a particular case of cancer are linked to form a single record

Death Registrations

Description: The Register includes all deaths that occur. Data is obtained from the Death Registration application form provided by funeral director and completed by spouse, partner or relative of deceased and Medical Certificate of Cause of Death and if a perinatal death, a Perinatal Supplement (to cause of Death Certificate), supplied by the certified medical practitioner certifying the death or supplied as a result of a coronial investigation.

Mental Health

Description: Data records information about mental health care including the assessment, treatment, recovery and care of child and adolescent, adult and older persons in non-admitted settings. It may include mental health prevention services and day programs, psychiatric outpatients and outreach services and hospital based consultation-liaison services to admitted patients in non-psychiatric and hospital emergency settings.

Perinatal

Description: Data records information about pregnancy care, services and outcomes including maternal demographics, antenatal care, labour, delivery and condition of child. It includes all births in public and private hospitals, and home births. It encompasses all live births and stillbirths of at least 20 weeks gestation or at least 400 grams birth weight.
